Radiation diagnosis of lung contusions at closed chest injury
Objective: To investigate the x-ray signs of lung contusion in patients with closed chest injury depending on the degree of the injury severity.
Material and Methods: The study involved 110 patients of Traumatic Shock Department who were performed survey radiography and fluoroscopy of the chest organs, dynamic x-ray investigation and computed tomography.
Results: Lung contusions were diagnosed in 59 patients (53.6 %), of them 25 (42.4 % )had areas of increased lung picture and small multiple, sometimes confluent, foci up to 3 lung areas, which disappeared after 2-5 days. The signs were considered grade I lung contusion. Twenty-three patients (39%) developed areas of uneven shadowing in the lungs over 3 lung areas disappearing within 10-20 days. The signs were assessed as grade II lung contusion. In
II patients (18.6%), total uneven due to discoid atelectases and subpleural lungruptures shadowing of the lung parenchyma, which resolved within more than 20 days, was revealed. The signs were assessed as grade III contusion. Grade II and III contusions developed in 24 patients with multiple fractures of the ribs (70.6 %), and in 25 cases of floating rapture (73.5 %).
Conclusion: One of the commonest types of lung injury at closed chest injury is lung contusion of mild, moderate and severe grade. Grade II and III lung contusions are accompanied by subpleural ruptures of the lung parenchyma and are often combined with multiple and floating rib fractures, which considerably aggravate the state of the patients.

Contemporary state of the problem of radiation lesions of the lungs: scientometric analysis
Summary. Scientometric analysis of the contemporary state and prospects of development of the problem of lung radiation lesion allowed to prove its urgency. But among separate questions, the most important is solving the problem of lung radiation toxicity. The majority of publications about clinical trials suggest about a constant search for new means of prevention and treatment. It was established that the information search efficacy about lung radiation lesions depended on the adequate use of term corpus of PubMed database.

The influence of total radiotherapy dose on the results of treatment for epidermoid lung cancer
Summary. Increase of total dose of radiation therapy up to 8086 Gy in patients with epidermoid lung cancer, in whom complete tumor regression was not achieved after the main course of treatment at a total dose of 64-66 Gy, allowed to achieve complete regression in 51.2 % of the patients and increase survival rate: one-year - form 44.3 ± 3.0 to 63.4 ± 7.5 %, three-year - from 6.5 ± 1.6 to 13.1 ± 5.4 % , five year - from 2.8 ± 1.2 to 10.5 ± 4.9 % .

Radiation-induced pathomorphism of infiltrative breast carcinoma at large-fraction gamma-therapy
Summary. The study involved 68 cases of infiltrating breast carcinoma at multimodality treatment (pre-operative gamma- therapy and surgery) within the period of 2006-2008. One third of the patients (33.8 %) demonstrated a positive therapeutic effect of pre-operative gamma-therapy with large fractions, i.e. radiation induced grades II-III, III, III-IV and IV pathomorphism.

Long-term results of cooperated investigation of neoadjuvant chemoradiation therapy efficacy in operable colon cancer
Summary. Radical surgery followed by chemotherapy in colon cancer (CC) does not guarantee relapse or distance metastases prevention. The effect of the standard for CC preoperative chemoradiation therapy (CRT) has not been investigated in this disease. We developed and applied in a cooperated investigation a technique of pre-operative CRT in patients with stage II-III CC, which is recommended to be introduced to practice.

Phase II investigation: partial breast irradiation with high-dose brachytherapy using intratissue multicatheter implant
Summary. Local control, side-effects, and cosmetic results were analyzed in patients with early-stage breast cancer after organpreserving surgery and adjuvant partial accelerated irradiation of the breast using high dose rate brachytherapy and intratissue multicatheter implant. The patients over 50 with solitary tumors < 3 cm invasive ductal carcinoma, differentiation grade I-III, resection R0, N0 (axillary dissection or investigation of signal lymph node) were included in the study. The irradiation was performed twice a day with a 6-hour interval at a single dose of 4 Gy. Total focal dose of 32 Gy was delivered with 8 fractions. With a mean observation period of 31 months (13-46), a local relapse was diagnosed in one patient (1.7 %). Cosmetic results were assessed as good and excellent. Immediate complications of the treatment were minimal. The method can be indicated in a selected group of patients and cannot be a standard of treatment at present.

Capabilities of sphingomyelinase in assessment of chemoradiation therapy efficacy in non-small-cell lung cancer
Summary. The results of radiation therapy with different fractionation modes and chemomodification with Etoposide for non-small-cell lung cancer (NSCLC) were analyzed. The objective effect proved to be significantly higher at application of accelerated regimens of radiation therapy and was accompanied by statistically significant increase of blood serum sphingomyelinase activity, which is a serous marker of ceramide pathway of apoptosis. It was established that the changes of sphingomyelinase activity in the blood serum correlated with the clinical results and could be used in prognosis and monitoring of chemoradiation therapy efficacy in NSCLC.

Comparative analysis and clinical assessment of efficacy of different methods of radiation therapy for malignant tumors of oral mucosa
Summary. Comparative analysis of radiation therapy in 45 patients with oral mucosa malignancies was performed. It was established that the use of non-traditional methods of radiation therapy significantly improved immediate and long-term treatment results.

Neoadjuvant chemotherapy for gastric cancer
Summary. The purpose of the work was to improve long-term results of treatment of patients with gastric cancer (GC) by means of working out and administration of neoadjuvant chemoradiation therapy (CRT). Fifty-two patients with GC aged 32 - 75 treated in 1999 - 2008 were analyzed. Prior to the surgery all patients were performed radiation therapy with a course of endolyphatic chemotherapy. The use of the developed method of neoadjuvant treatment for GC allowed to achieve mean duration of life of the operated patients 3.58 ± 0.69 years, 5-year survival 46. 0 ± 3.2%.

Neoadjuvant chemoradiation therapy in multimodality treatment for esophageal cancer
Summary. A program of randomized study of neoadjuvant chemoradiation therapy in multimodality treatment for esophageal cancer is presented. Radiation therapy was delivered against a background of 5-fluorouracil administration with the purpose to promote radiosensibilization and influence the tumor. The obtained findings suggest about a high efficacy and good tolerance of endolymphatic chemoradiation therapy.

The experience of radiation treatment for esophageal cancer using modification with 5-fluorouracil
Summary. The authors report the results of radiation treatment for esophageal cancer using modification with 5-fluorouracil at Ivano-Frankivsk Regional Cancer Hospital. It is shown that the use of split distant gamma-therapy with different modes of irradiation and simultaneous administration of a modifier 5-FU allowed to improve the efficacy and quality of the treatment, eliminate dysphagia faster and more stably as well as to improve the results of the treatment itself and survival indices.

Clinical cytogenetic aspects of occupational respiratory system cancer in workers of uranium mines of Mining and Enrichment Combine Skhid
Summary. Clinical morphological peculiarities and cytogenetic effects of respiratory system cancers were investigated in miners of uranium mines with the purpose to assess radiation influence of occupational harmful factors.
It was established that occupational cancers of the lungs were characterized by late stages of their diagnosis, development against a background of chronic obstructive lung diseases (COLD), histological peculiarities, (chiefly small-cell cancer), presence of a wide spectrum of accompanying diseases, first of all COLD and locomotor system diseases.
The revealed clinical morphological peculiarities of lung cancer can be additional criterion of professional expertise of cancers in workers of uranium miners.

The efficacy of neoadjuvant radiation therapy with chemomodification using 5-fluorouracil in elderly patients with local breast cancer
Summary. Clinical morphological substantiation of efficacy of chemomodification with intravenous and endolymphatic (EL) administration of 5-fluorouracil (5-FU) and radiation therapy rhythms in elderly patients with breast cancer was substantiated.
The efficacy of medium-fraction RT modified with sensibilizing doses of EL 5-FU was shown to be higher than that of classic RT and RT with intravenous 5-FU administration.

Radiation therapy with intra-arterial chemotherapy for oropharyngeal cancer
Summary. Regional intra-arterial chemotherapy for oropharyngeal cancer supplemented by intra-arterial antibiotic therapy improve the efficacy of radiation therapy due to reduction of the inflammatory component, the tumor regression and creates favorable conditions for better results. The use of regional intra- arterial chemo- and antibiotic therapy with radiation therapy allows to improve immediate results of the treatment as well as the quality of life of the patients with this severe disease.

Neutron sources for radiation oncology
Summary. The authors discuss the peculiarities of biologic effect of fast neutrons from the perspective of overcoming radioresistance of the tumor cells. Retrospective radiobiological and clinical data obtained during treatment of cancer with fast neutrons at Oncology Institute of Academy of Medical Sciences of Ukraine (now National Cancer Institute of Ministry of Health of Ukraine) are reported. It was proven that neutron therapy allows to achieve a positive effect in patients with primary tumors and relapses.
252Cf neutrons indicated to treatment of the tumors poorly sensitive to gamma-radiation were used. 252Cf sources were used in combination radiotherapy for cancer of endometrium and uterine cervix. The sources were introduced to the cavity and fornix of the vagina. The treatment was performed by means of alternation of distance and intracavitary radiation therapy. Complete tumor regression was achieved in almost all patients. Radiation reactions were easily controlled during the treatment. Investigation of longterm results of treatment with 252Cf sources demonstrated that their application allowed to achieve a high and prolonged relapse-free effect.

Improvement of chemoradiation treatment for squamous-cell cancer of anal canal
Summary. The authors describe the methods and techniques of radiation therapy at treatment of squamous cell cancer of anal canal, i.e. the use of radiation therapy before surgery and as a separate method of treatment. The use of modifiers with the purpose to change selectively radiosensitivity of the normal and tumor tissue and to increase the effect of the irradiation on the tumor is indicated. A new method of treatment for squamous-cell cancer of anal canal consisting of combination of radiation therapy effect with modifying effect of Xeloda and Phthorafur and proapoptotic, antiproliferative, and antimetastatic effect of Laferon was worked out. The results of conservative treatment of patents with small- cell cancer of anal canal as well as the data about the method toxicity are reported.

The use of chemoradiation therapy in primary vaginal cancer
Summary. A technique of complex treatment for local vaginal cancer using methotrexate lymphotropically, Laferon intramuscularly and Phthorafur or Capecitabine orally was worked out. The treatment was administered to 29 patients with stage II - III T2-3 N0-2 M0 vaginal cancer. Multimodality therapy was administered using a standard technique. Radiomodifying effect of oral Phthorafur or Capecitatine was used. The analysis of immediate results of the suggested techniques of chemoradiation treatment for vaginal cancer suggests about a higher effect of these two techniques when compared with the standard ones. Neither the number nor the degree of general and local radiation reactions increased during the treatment.

The role of chemomodifiers in radiation therapy for local breast cancer
Summary. The authors describe the association of treatmnt efficacy in breast cancer (BC) both with the use of methods of early tumor detection and multimodality therapy improvement. To increase the tumor damage with ionizing radiation and reduce radioresistance of the malignant cells, various physical and chemical methods, i.e. modifiers, are used. Simultaneous application of several modulation techniques (polymodification) is frequently used. The study involved 75 patients with BC. The patients from the main group (34 persons) were administered radiation therapy against a background of cytostatic drug (Phthorafur) administration in modifying doses and inductothermy. The controls (41 patients) were administered radiotherapy only. Significant increase of the tumor regression at local BC was demonstrated when radiotherapy was delivered against a background of modifiers administration. Local and general reactions did not exceed grade II according to RTOG/EORTC, 1995.

Conservative treatment for local cervical cancer using cytostatics and interferons against a background of combination radiotherapy
Summary. Combination radiation therapy was performed using the radical protocol in 53 patients with stage IIB-IIIB cervical cancer, of them 23 were administered Phthorafur® in radiomodification mode: 1 mln/day during the treatment. Dynamic clinical laboratory, instrumental investigations during the treatment revealed significant increase of the frequency and degree of the tumor regression at inconsiderable increase of side- effects and positive dynamics of the immunological profile, which was considered a positive prognostic sign.

The experience of chemoradiation therapy application in local cervical cancer
Summary. The experience of administration of chemoradiation therapy in patients with stage IB-IIIB cervical cancer is presented. Combination of radiation therapy with Xeloda in modifying doses is shown to allow to improve the efficacy and quality of treatment in this group of patients without increase in toxic effects. When paraortic and paracaval lymph nodes involvement is revealed in different terms after the treatment, paraortic and paracaval lymphadenectomy followed by chemotherapy is indicated.

Immediate results of chemoradiation therapy for local laryngeal cancer using accelerated hyperfractionation with tumor dose escalation
Summary. Immediate results of chemoradiation therapy (CRT) for local laryngeal cancer (LC) with the use of accelerated hyperfractionation with tumor dose escalation were compared with those of standard RT. Immediate CRT results of accelerated hyperfractiobation with staged dose increase in the tumor significantly higher when compared with RT in traditional fractionation mode. The mode was most advantageous in patients with stage IV LC with metastases to the cervical nodes. The original method of conformal CRT in dynamic accelerated hyperfractionation with gradual dose increase in the tumor did not cause increase of incidence of severe radiation reactions in the mucous membrane of the oropharyngeal area and laryngeal cartilages.

Investigation of zoledronic acid properties as an anti-tumor drug
Summary. The authors report thefindings of primary clinical trial of zoledronic acid as an anti-tumor (antimetastatic) drug. The monitoring involved 14 women with receptor-positive breast cancer without metastases to theskeleton, who, beginning from 2007, were administered Zometa 2 times a year. The controls were 17 patients with IIA and IIB cancer. Promising results of the treatment which did not demonstrate metastases to the bones after the surgery and adjuvant chemoradiation and hormonal therapy were obtained.

Summary. Main method of treatment for non-small-cell lung cancer (NSCLC) is multimodality therapy including surgery and chemoradiation therapy (CRT). The authors report the results of 70 organ-saving operations. According to the literature, radical organ-saving operations in NSCLC can be a method of choice when combined with CRT. The indications to pre-operative and postportative CRT in this group of patients require further investigation and elaboration.

Efficacy of cardiovascular complications correction in patients with breast cancer
Summary. The work was performed with the purpose to assess the efficacy of cardiovascular complications correction at combination treatment for breast cancer (BC). Timely diagnosis and correction of cardiovascular diseases in BC with the use of inhalation cardioactive drugs (nitrates and calcium antagonists) improved the efficacy of accompanying therapy, prevented progress of early and late RT complications, improved the quality of life.

The results of radiation therapy or observations in patients with stage I testicular seminoma
Summary. The results of treatment and incidence of relapses of 238 patients with stage I testicular seminoma after radiation therapy (RT) and only observation tactics were analyzed.
Long survival of patients with stage I seminoma is high. Both tactics (adjuvant RT and observation) give long survival. RT results in less number of relapses. The advantages of observation tactics are absence of radiation side-effects. The task consists in identification of prognostic factors promoting relapse development.

Summary. A method of intra-arterial chemosensibilization with radiation therapy developed at Donetsk regional antitumor center is described. The role of radiation therapy in multimodality treatment for renal-cell cancer
Summary. The authors report the results of multimodality treatment for renal-cell cancer (рТ any, N any, M0) using pre- operative large-fraction irradiation. Our findings demonstrate that radiation therapy does not aggravate the conditions for surgery and improves long-term results.
The data about efficacy of multimodality treatment (palliative nephrectomy with radiation therapy) in patients with primary metastatic kidney cancer ^Т any, N any, M1) are also reported.

Chemoradiation therapy for primary inoperable cervical tumors
Summary. A technique of chemoradiation therapy for advanced forms of cervical cancer (CC) is featured. The performed investigation demonstrated that the use of chemotherapy with antibacterial therapy before combination radiotherapy in patients with primary inoperable CC promoted improvement of immediate treatment results as well as reduced the incidence of radiation complications, thus improving the quality and duration of life of the patients.

Radiation therapy for liver metastases using linear accelerator
Summary. The capabilities of radiation therapy for liver metastases using a linear accelerator are discussed. It was established that delivery of the irradiation dose to the area of metastases > 40 Gy not only produced pronounced symptomatic effect but also increased life span of the patients with this pathology.

Pathomorphological features of dedifferentiatedcervical cancer before and after stage 1 of combined radiation therapy
Summary. The pathomorphological characteristics of undifferentiated cervical cancer (CC) were investigated before and after stage 1 of split two-stage combined radiation therapy. The developed combined method of treatment for local CC allows to achieve positive dynamics of the cervical tumor pathomorphism, which manifests by reduction of the parenchyma volume and mitotic index as well as increase of apoptosis index.

Chemoradiation treatment for local breast cancer
Summary. This analysis of the method of treatment for local breast cancer suggests that complex approach does not reduce the probability and severity of the post-operative complications course but is a highly effective and maximally safe method aimed at inhibition of the growth and dissemination of possible subclinical metastases as well as reduction of the size of the primary tumor, which allows to perform surgery.

Post-operative radiation and chemoradiation therapy with Temozolamide in patients with glioblastomas
Summary. Combination of post-operative radiation therapy (RT) at a TFD 69 Gy and Temozolamide (TM Z) at a dose of 75 mg/m2 1 hour before irradiation during the first and last 2 weeks of RT followed by administration of 6 courses of TMZ at a dose of 150-200 mg/m2 from day 1 to day 5 increased (when compared to post-operative RT) median and 3-year survival of the patients with glioblastoma from 10 to 23 months and from 3 to 28%, respectively (p = 0.002).

The influence of optic radiation on the state of the system of homeostasis in patients with breast cancer during radiation therapy
Summary. The influence of phototherapy with red and blue light as well as their combination on the state of homeostasis in patients with breast cancer was investigated during the course of postoperative radiation therapy. It was established that phototherapy possessed multisystemic effect and positively influenced the state of homeostasis system with all schemes of optic treatment. The most pronounced was the effect of blue light as well as its combination with red.

The ways of optimizing the treatment for local rectal cancer
Summary. The treatment of the patients who were administered pre-operative multimodality treatment using two techniques was analyzed.
In group 1, 18 patients were treated using open fields with intracavitary therapy on the first stage. In group 2, all 9 patients were administered 3-field treatment. In all patients from group 1 the tumor was transferred to an operable form.

Comparative efficacy of orthopantomography and helical computed tomography in diagnosis of the jaw osteonecrosis in patients with myeloma disease
Summary. The purpose of the work was to investigate informativity and efficacy of radiation diagnosis of osteonecosis of jaw bones in patients with myeloma disease treated with bisphosphonates (BP), using orthopantomography and helical computed tomography (HCT) of the jaws.
The study involved 53 patients with MD who together with chemotherapy were administered bisphosphonates (31 women, 58.5 % and 22 men, 41.5 %). Mean duration of therapy was from 6 to 57 months. The state of the lower jaw was investigated using orthopantomography and helical computed tomography.
Orthopantomography allowed to reveal the foci of osteonecrosis in the lower jaw, HCT allowed visualizing various grade manifestations of osteonecrosis.

The analysis of early and late radiation lesions at multimodality treatment for laryngeal cancer
Summary. The work deals with the immediate treatment results in patients with laryngeal cancer who were administered radiation therapy in combination with neoadjuvant selective intraarterial chemoembolization of the tumor. The treatment outcome of 103 patients was analyzed. Application of the developed method improved the efficacy of the treatment due to reduction of the manifestations of the late radiation lesions. Chemoembolization produces not only radiosensitizing effect on the tumor but also protects the normal tissues.

Efficacy of magnetoacustoradiation treatment of experimental М-1, РС-1 tumors
Summary. The urgency of the struggle against malignant tumors is determined by a stable growth of cancer incidence, high level of disability and morbidity, the cost and difficulty of treatment. The influence of low-frequency ultrasound and alternating magnetic field on transplanted tumors of sarcoma M-1 and alveolar liver cancer PC-1 together with radiation therapy at a SFD of 5 Gy and 20 Gy was investigated in the experiments on animals.
It was established that the influence of the above physical factors inhibited sarcoma M-1 and alveolar liver cancer PC-1 growth, prolonged the life and survival of the animals of the investigated groups when compared to the intact controls, which definitely expanded the ideas about the capabilities of ultrasound and magnetic fields in cancer treatment.

The influence of low-dose ionized radiation on the marker of astroglial population of the brain cells (Experimental investigation)
Summary. The peculiarities of astrocyte response caused by single and fractionated irradiation of rats at a dose of 0.25 Gy during post-radiation period were investigated. Single irradiation was established to elevate soluble glial fibrillar acid protein (GFAP) 12 hours after irradiation in the hippocampus and corpus striatum and on day 5 in the midbrain when compared to the controls. Fractionated irradiation demonstrated less pronounced reverse changes of GFAP amount when compared with those at single irradiation. Soluble GFAP increased in the brain neocortex and reduced in the hippocampus and corpus striatum. Our findings show that single and fractionated irradiation at a dose of 0.25 Gy causes the changes in the amount of soluble GFAP in different structures of the brain of the rats during radiation exposure.
